ENROLLMENT Request Form

To request to enrollment in Biology 1A and/or 1AL, follow these instructions for filling out this form.

Fill out this form completely and submit it by 3:00 PM Monday 6/23. This form will be disabled after that deadline.

You will be contacted either by email or phone to inform you of the status of your request.

Do not try to attend a lab or discussion section unless you have been informed that you have been selected to be enrolled in the course(s). This will not improve your chance of being enrolled.

If you find that you have made a mistake after submitting or your schedule changes causing your choices to change, just submit the form again with the correct information. We will use the most recent submission.

In addition to filling out this form, you must also be on the waiting lists for the class(es) you are requesting to enroll. You should have selected discussion and/or lab section #100.

If you are selected to be enrolled, you will be enrolled into the discussion/lab section that was open and matched up with the selections you indicate later in this form. You will also be infromed of your enrollment by email.

The enrollment status of the course will be checked periodically and students will be enrolled from the waiting lists if there is any open space.

LAB and/or DISCUSSION SECTION SELECTION:

Look below at the scheduled section times. The choices are for the times that the sections meet not a specific section number. If you are requesting enrollment in only 1A ore only 1AL, your preferences below will only be counted for Discussion or Lab accordingly. Use the menus to indicate your preference using 1 for most desired to 4 for least desired. Select as many as possible to increase your chances of getting enrolled if there is space. Sections 103 and 104 are the most likely to have space to add students. If you are requesting to enroll in both courses, you must be enrolled in the same numbered discussion and lab sections. You cannot request to mix sections.
